What you’ll be doing in this role
Case Managers are part of the Clinical Care Coordination team and an integral part of the multidisciplinary team for ensuring that all patients are supported throughout their hospital journey, with a particular focus on care during the complex discharge planning period.
Your key responsibilities will include:
- Delivering exceptional evidence-based nursing care and monitoring to patients
- Overseeing all patients within their designated specialty areas, focusing on patients’ length of stay and accurate clinical documentation
- Ensuring ease of access and smooth transition between points of care
- Facilitating quality patient care
- Contributing towards excellence in care management
- Contributing towards the ongoing development of revenue improvement
- Wholeheartedly contributing to a culture of clinical excellence and continuous quality improvement; and
- Actively contributing to a culture of comradery and teamwork
Who we’re looking for
- You’ll bring excellent clinical skills, with minimum three years clinical experience
- You’ll have recent experience in Discharge Planning / Case Management
- Your friendly and positive way of working with people will be obvious in how you considerately and professionally communicate with patients, their families, visitors, and your colleagues
- You’ll have experience in Microsoft Excel and databases
- You’ll be collaborative, adaptable, and be able to work independently and in a team
- You’ll be very committed to contemporary nursing practice and individual patient-centred care
- You’ll be empathetic, systematic, detail-oriented, and prepared to embrace Adventist Healthcare’s everyday values of excellence and integrity
- You’ll hold a nursing degree with current AHPRA registration, and
- Ideally, you’ll also have some community nursing experience or recent experience in Palliative Care
- Demonstrated ability to conduct research and demonstrated knowledge of health funds would be welcome
- Membership to the Case Management Society of Australia would be desirable
- Preferably, you will have also completed the NSW Health DETECT, Between the Flags Program and Bloodsafe Clinical Transfusion Practice programs.
