What you will be doing in this role
We are seeking a compassionate, adaptable and experienced Social Worker to join our multidisciplinary team in a casual capacity. This is an excellent opportunity for a skilled clinician who enjoys the variety and fast pace of a hospital environment and is passionate about supporting patients and their families through the challenges they face during hospitalisation and discharge.
By collaborating with relevant multi-disciplinary team members, you will lead and coordinate the safe transition of patients back home. You’ll communicate daily with the care team about new admissions and upcoming discharges and recommend services to meet patients' healthcare needs. You’ll play a key role in liaising with patient families and initiate conferences and action plans.
This is a Casual position to cover ad-hoc leave for the team. Shifts are Monday-Friday between 8am and 4.30pm.
Who we’re looking for
- You'll have a Social Work degree from a recognised university.
- Eligibility for membership with the Australian Association of Social Workers (AASW).
- Recent experience in discharge planning within an acute care setting.
- Strong communication skills, with the ability to engage professionally and compassionately with patients, families, visitors and colleagues.
- A collaborative and adaptable approach, with the ability to work effectively both independently and as part of a multidisciplinary team.
- An empathetic, organised and detail-oriented work style, along with a commitment to upholding Adventist Healthcare's values of excellence and integrity.

What to expect
